Your first visit to Little Harpeth Children's Dentistry™ involves a few special steps so that we can get to know you. To understand what to expect, please read through this page. You'll find all the practical information you need, such as a map and directions to our office, practice hours, payment policies and more. Please note ALL children seen must be FULLY vaccinated AND a birthparent must accompany them to the initial visit.

Patient Forms

Please complete the emailed/ texted forms in advance so we can expedite your first visit. It is REQUIRED that New Patient Paperwork is completed 3 days prior to your initial appointment. Failure to do so may result in the inability to see you at your scheduled time. Children are REQUIRED to me fully immunized to be seen here (ie. MMR, Tetnus). In todays climate, it has become harder for us to verify your dental insurance policy in a shortened time span and please be reminded that we do this as a courtesy. You are always responsible for what your dental policy does not cover as you purchased it.

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. Educate yourself on your symptoms by reviewing the content on this Web site.

Masks are recommeded to be worn by ALL accompanying adults, and ALL children seen must be up to date with their immunizations.
